If you have not booked, you can book a virtual, TeleMed visit using the button above. After we send you a link, call us to pay your co-pay and deductibles or make prior arrangements with our clinic to bill your insurance and have a credit card on file as a backup payment.

Our staff and providers are all vaccinated with Covid-19 vaccine. But, for your safety and the safety of other patients who have not been vaccinated, if you have Covid-19 like symptoms such a cough, cold, shortness of breath or fatigue you can only be seen via TeleMed video conferencing. After your TeleMed visit is complete, an in-clinic visit may be scheduled as needed.

Preparing For Your Tele-Medicine Video Visits

We use a HIPAA compliant TeleMed virtual visit application integrated with our EHR & Practice Management Office Ally System to facilitate the workflow of our providers to serve you better.

After you have provided your insurance info and your email we will schedule a TeleHealth visit. Self pay and not insured or insured with high deductible plans are welcomed. TeleMed visits are affordable and have transparent pricing. We will send you a confirmed TeleMed appointment notice; our application will send it via an email and/or SMS notification 15 minutes before the scheduled meeting time. TIPS:

  • Have adequate lighting.
  • Make sure you are sitting in a lighted environment so that the doctor can see your face and body well.
  • Have a flashlight with you. The doctor may ask you to zoom the flashlight and your camera at certain parts of your face or body.
  • Have your chief complaint, reason for visit, medical history and questions or concerns you may have for the doctor.
  • If you need privacy with our provider, make sure you are in a place that can facilitate it.

Start Your Visit

Click on the link sent to you by our healthcare provider, start the video conference and join the meeting with your provider. Make sure you share your video and audio; otherwise our provider will not be able to see you or hear you. Please join 5 minutes before and wait for your provider to join. If you have any difficulty joining the TeleMed consult or your provider has not joined on time, please feel free to call us at (408) 945-0300 to verify your appointment and/or when the provider will be able to join you.

What to Expect During and After Your TeleMed Video Visit

The Doctor Will Need Your Medical History

Please take your temperature, height, weight & pulse ahead of the exam time.

Be prepared to share your medical history, chief complaint, your weight and height, etc. You may be asked to zoom with your camera, and if needed with a flashlight, to look at the part of your body with affected areas. Make sure you have privacy. Your home is your exam room.

Make a list and be prepared to ask your questions and share your concerns so they can all be addressed.

After the Visit

The doctor will send your Rx or orders for lab or radiology and set up a follow up visit to discuss the outcome of results and plan treatment based on your results.

How Do I Book an Appointment?

Select the reason for appointment/service, up to 2 convenient dates and times, provide the required info and submit your request. This is just a request for a tentative appointment booking and is not confirmed. We will confirm your appointment within 24 hours by voice, email or SMS text. If urgent, please call our clinic at (408) 945-0300 during clinic open hours.
