We try to see all patients on an appointment basis, and we request that you set an appointment so we can reserve time for you. The office telephone number is (408) 945-0300. We make every effort to honor all time commitments and request that you extend the same courtesy to us. On occasion emergencies can cause demands on the physician’s time, and whenever possible, you will be fully informed if there will be any delays because of this.

Online 24×7 Access: You can request to Book Appointments online anytime. If you are an existing patient and have enrolled in our Patient Portal, you can schedule appointments, view your medical records & immunizations and communicate with our clinic staff. You can always contact us online and send us a message.

What are Clinic Hours?

Appointments for routine problems and follow-ups are available on the following schedule. Both TeleMed and In-Clinic appointments are available. Due to Covid-19, schedules may vary.

Appointments

We try to see all patients on an appointment basis, and we request that you set an appointment so we can reserve time for you. The office telephone number is (408) 945-0300. We make every effort to see walk-in patients without appointments, in particular if the patient is sick and needs attention. For routine physicals and school physicals, please plan and make an appointment. You can also Make Appointments online & we will confirm your request for an appointment.

Online Booking of Appointments is available 24×7 in the Appointment Tab. Walk-ins are welcome and we will work you into the schedule.

Do You Send Appointment Reminders?

Yes, we remind you of your appointments depending upon your preference via email, your cell phone, text or a call to your home phone. You can Book Appointments online. You can also enroll in Patient Portal to schedule appointments online, request refills, view your medical records and pay bills online.

You can leave non-urgent messages for things such as making appointments, billing questions, making payments, insurance info, referrals, Rx refills, copy of medical records or call during normal clinic hours between 9 am to 6 pm.

Telephone Calls

All patients are encouraged to call with any questions they have on medical problems. However, it would be unfair to our patients if the doctor had to leave to answer every telephone call. The office staff have been trained to answer many of your questions. They will also relay your information to the doctor and your call will be returned when convenient or otherwise answered at the earliest opportunity. For any medical care, it is better to make an appointment so that you can discuss matters concerning you or your dependent family members in person.

After Hours Telephone Calls for Doctors

If you find it necessary to contact the doctor after normal office hours, the telephone answering system will prompt you to call the doctor and leave a message. All messages go to your doctor or the physician on call. Your call will be returned at the earliest opportunity. If it is an emergency, call 911 and/or go to the nearest emergency room. If minor urgent care is needed, you can go to an urgent care facility near you. Our physicians will take care of you the next day.

Prescriptions and Renewals

We use e-prescribe and Electronic Health Records (EHR). If you have enrolled in our Patient Portal, you can request refills and/or make appointments online.

All prescriptions and authorizations for renewals should be requested during normal office hours. Rx fills are usually not considered an emergency.

You are encouraged to make an appointment with the doctor to review your medical condition and assess the necessity for Rx renewal, adjustment of the dose, discontinuing the medication or changing the medication.

Fees, Payments and Insurance Coverage

Fees and Payments

We make every effort to keep down the cost of your medical care. You can help by paying upon completion of each visit. Other arrangements can be made with our office manager depending upon special circumstances.

You are responsible for any unpaid charges by your insurance.

Your policy has certain deductibles, co-payment and patient share of expenses. Insurance policies and coverage are complex. Not all procedures, vaccinations, exams and treatments are covered by insurance. It is your responsibility to make sure you are eligible and determine what is covered or not and what is your share of the cost. We can answer some questions and help process claims, but ultimately you are responsible for any unpaid bill.

Insurance and Insurance Forms

We try to simplify the preparation of insurance claims, thereby holding down costs that are unrelated to the delivery of good medical care.

Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charges. It is your responsibility to pay any copayment, deductible amount, co-insurance, or any other balance not paid for by your insurance company.

We know questions can arise on insurance matters and these should be discussed with our insurance clerk. We will be happy to help you receive maximum benefits; however, the agreement of the insurance company to pay for medical care is a contract between you and the company.

House Calls

Home visits are virtually impossible because of the lack of equipment at home and the time-consuming problems which prevent other patients from receiving medical attention.

We see walk-in patients. We work you in and our dedicated staff try their best to accommodate you depending upon the medical urgency. You may have to wait as patients with appointments have priority.

You are encouraged to make an appointment, but in case you do not have an appointment and you need to be seen, we will do our best.

Who Does the Consultations and Performs the Laser Procedures?

Dr. Shivani Chawla Mehta and her staff perform the majority of these treatments.

Melissa-Gayle Sanchez M.D. and Shivani Chawla Mehta M.D. are board certified physicians, practicing pediatrics, allergy and family medicine. Dr. Chawla Mehta practices a holistic approach to health and uses supplemental holistic medicine with conventional medicine.

Do You Have FREE Wi-Fi in Your Office? - Yes

Yes, we do have Free Wi-Fi for our patients while you are in the office. Ask the receptionist for access. In addition, we have magazines and educational videos in the waiting room. Usually, your wait in the office should be less than 15 minutes after you have checked in and filled out paperwork.
